Our Health and Community Learning Collaboratives / Fellowships are 12–18 month cohort-based programs that develop and expand organizational capacity for results-oriented, project-centered leadership. Fellows receive coaching and instruction in population health, quality improvement, stakeholder engagement, and systems change, then apply what they’ve learned to real-world initiatives that have measurable impact on clinical and community outcomes.

A Health Equity Impact Assessment (HEIA) is a thorough, data-driven examination of how a policy, program, or facility change would affect healthcare access and outcomes, particularly for medically underserved communities. In New York State, the HEIA is a mandatory part of the Certificate of Need (CON) process for certain healthcare facilities under Public Health Law §2802-b (effective June 22, 2023). As a certified “independent entity,” SmartRise Health assists organizations in meeting legal obligations, engaging stakeholders, and developing meaningful, community-focused solutions.
